Individual Student Planning...

is the process of determining the services and supports that a student
with disabilities needs in order to access all instructional, social,
and extra-curricular activities within his/her learning environment
and insure that the student’s individual educational plan (IEP) is
integrated into the instruction. Through a systematic planning
process, the support team determines how the student will access the
instruction and curriculum. The stakeholders surrounding the student
make up the support team. The support team consists of the teachers,
parents, paraprofessionals and support staff surrounding the student.
The support may also include community representatives if the
student’s learning environment extends to the community in the case of
community-based instruction, job placement or supported employment.
The support team along with the student determines how best to enable
the student to access learning across the school day through the use
of adaptations and support.
